There already was a macro PageGetItemId; this is now used in (almost)
all places, where pd_linp is accessed. Also introduce new macros
SizeOfPageHeaderData and BTMaxItemSize.
This is just source code cosmetic, no behaviour changed.
All 81 tests passed.
