Just for info, if name is empty, then probably there will be no
parameter 8, etc.
Use following block:
int i=2;
if (!name.isEmpty()) {
stmt.setString(i, "%" + name + "%");
i++;
}
etc...
stmt.setInt(i, pageOffset);
