Avoid <substeps> element in man pages

The upstream DocBook manpages stylesheet apparently does not handle
the <substeps> element at all, and so the content comes out
unformatted, which is not useful.

As a workaround, replace <substeps> with a nested <procedure>, which
ends up effectively the same in output.
