So, would anyone be averse to something like the following:
ALTER TABLE blah ADD ... PRIMARY KEY (...) USING PREBUILT INDEX index_hame
If the user doesn't specify CONSTRAINT constraint_name, it will
default to current implicit behavior of col_pkey.
