I have cleaned up the SSL -is mess. Flag is no -l, with check that if
they use -l, they also must use -i or they get an error message on
startup.

I fixed some -d debug handling that was broken in postmaster and
postgres programs.

Another PERL variable name fix.

Documentation updates.
