Doesn't
SET TRANSACTION ISOLATION LEVEL SERIALIZABLE;
have to come within transaction - ie
BEGIN;
SET TRANSACTION ISOLATION LEVEL SERIALIZABLE;
In this order your test seems to behave correctly - only 1 row inserted.
